Diabetic patients on chronic disease programme show improvement

SINGAPORE: Diabetic patients who stayed on the Chronic Disease Management Programme for two years have shown significant improvements in their diabetic, lipid and blood pressure control.

There are currently nearly 100,000 patients on the programme — most of them are over the age of 50, with multiple chronic conditions.

This represents a five—fold increase in patient numbers since the programme started three years ago.


The programme aims to encourage patients with chronic conditions — such as diabetes, asthma and hypertension — to work closely with their family doctors to better manage their diseases.


Speaking at the opening of the 6th Health Technology Assessment International Annual Meeting on Monday, Health Minister Khaw Boon Wan said the results of patients here are comparable or better than developed countries, such as the United Kingdom and the United States.


He added that data collected from the programme over the past two years will be published online.
