Insulin breakthrough could see end to needles

SYDNEY: Breakthrough Australian research mapping how insulin works at a molecular level could open the door to novel new diabetes treatments, ending daily needle jabs for millions, scientists have announced.

A Melbourne team have been able to lay out for the first time how the insulin hormone binds to the surface of cells, triggering the passage of glucose from the bloodstream to be stored as energy.

Lead researcher Mike Lawrence on Thursday said the discovery, more than 20 years in the making, would make new and more effective kinds of diabetes medication possible.

"Until now we have not been able to see how these molecules interact with cells," said Lawrence, from the Walter and Eliza Hall Institute of Medical Research.

"We can now exploit this knowledge to design new insulin medications with improved properties, which is very exciting."

Lawrence said the team's study, published in the latest edition of Nature, had revealed a "molecular handshake" between the insulin and its receptor on the surface of cells.

"Both insulin and its receptor undergo rearrangement as they interact -- a piece of insulin folds out and key pieces within the receptor move to engage the insulin hormone," he said of the "unusual" binding method.

Understanding how insulin attaches to cells was key to developing "novel" treatments of diabetes, a chronic condition in which the pancreas does not produce enough insulin or the body cannot use it properly.

"The generation of new types of insulin have been limited by our inability to see how insulin docks into its receptor in the body," said Lawrence.

"This discovery could conceivably lead to new types of insulin that could be given in ways other than injection, or an insulin that has improved properties or longer activity so that it doesn't need to be taken as often."

Importantly, he said it could also have ramifications for the treatment of diabetes in developing nations, allowing for the creation of a more stable insulin that does not need refrigeration.

There are an estimated 347 million diabetes sufferers worldwide and diagnoses are increasing, particularly in developing countries, due to growing levels of obesity and physical inactivity.

It is expected to be the seventh leading cause of death in the world by 2030, with the World Health Organisation projecting total deaths from diabetes will rise by more than 50 percent in the next 10 years.

Complications of diabetes include blindness, limb amputation and kidney failure.

Doctors in Britain looked at five studies in which children were monitored from infancy to early childhood to see if vitamin D supplements made a difference to the risk of becoming diabetic.

The risk of developing the disease was reduced 29 percent in children who took extra vitamin D as compared to those who had not.

Diabetes is a chronic condition in which the body does not produce enough of the hormone insulin, or cannot make proper use of the insulin it does produce, a condition called insulin resistance.

In Type 1 diabetes, so-called beta cells in the pancreas that produce insulin are destroyed in early childhood by the body's immune system.

The disease is most common among people of European descent, affecting around two million Europeans and North Americans, and for reasons that are unclear is becoming more widespread.

Type 2 diabetes, which is far more common, is linked mainly with an unhealthy diet and sedentary lifestyle. It is becoming epidemic in scale in many developed or fast-developing countries.

My 12-year-old daughter has a fatigue issue. She feels lethargic all the time and requires more than 10 hours, at times 14 hours, of rest. She is not overweight and does not snore when she sleeps, so I have ruled out poor-quality sleep. When she was less than eight months old, she rolled and fell from the bed. Most girls her age have reached, or are reaching, puberty, but she has yet to experience any changes. I wonder if these factors are causing her to be tired all the time. Or, could she be suffering from some underlying medical problems?

The fall she had when she was eight months old is unlikely to have caused her current symptoms. And a proportion of young girls start puberty only when they are 12 to 14 years of age. Obesity and poor-quality sleep caused by an obstructed airway can also contribute to lethargy and tiredness, but in her case, it is less likely.

Other causes of prolonged fatigue and tiredness could be anaemia, either from blood loss or decreased production, or poor dietary intake; infections — some, such as dengue, can cause prolonged lethargy and tiredness; hormonal problems, such as hypothyroidism or diabetes mellitus; chronic illnesses, such as kidney or liver diseases; lack of exercise or excessive physical exertion.

Stresses in balancing school work, CCAs and social/recreational activities could also lead to anxiety, depression, withdrawal from social contact and, consequently, chronic fatigue.

Sometimes, there may be no underlying cause found after extensive investigations, and a diagnosis of chronic fatigue syndrome, which could inflict a child for a prolonged period of time, may be offered.

Consult a doctor to ensure that any underlying medical problem is excluded and appropriate lifestyle changes instituted to get the child back to her normal, healthy state. Think a balanced diet with sufficient fruits and vegetables, regular and appropriate exercises, and a conducive school and home environment for mental and social well-being.

BRISBANE - An American doctor was accused yesterday of scheduling unnecessary and dangerous operations that prosecutors said resulted in the deaths of three patients and the serious harm of another.

Indian-born surgeon Jayant Patel pleaded not guilty to three charges of manslaughter and one count of grievous bodily harm in a crowded courtroom at Brisbane Supreme Court.

His trial is expected to take four to six weeks and hear testimony from some 90 witnesses. The trial comes more than 25 years after questions were first raised about Patel's competency, and five years after a government inquiry found he may have directly contributed to patient deaths.

Patel, 59, has not spoken publicly about the charges, which relate to four patients he treated while working as director of surgery between 2003 and 2005 at a state-run hospital in Bundaberg, a sugar industry town. He faces a possible sentence of life in prison if convicted. The case has received massive media attention throughout Australia.

Prosecutors say Patel repeatedly performed surgeries he had been banned from undertaking in the United States, misdiagnosed patients and used sloppy, antiquated surgical techniques.

In his opening statement, prosecutor Ross Martin said the fact the patients consented to the operations was irrelevant. "To put it shortly and crudely, one cannot consent to one's own death or grievous bodily harm," he said.

In 1984, Patel was fined by New York health officials and placed on probation for three years for failing to examine patients before surgery. He later worked at Kaiser Permanente Hospital in Oregon. Kaiser banned him from liver and pancreatic surgeries in 1998 after reviewing 79 complaints about him. The Oregon Board of Medical Examiners later cited Patel for "gross or repeated acts of negligence". AP
