Insulin breakthrough could see end to needles

SYDNEY: Breakthrough Australian research mapping how insulin works at a molecular level could open the door to novel new diabetes treatments, ending daily needle jabs for millions, scientists have announced.

A Melbourne team have been able to lay out for the first time how the insulin hormone binds to the surface of cells, triggering the passage of glucose from the bloodstream to be stored as energy.

Lead researcher Mike Lawrence on Thursday said the discovery, more than 20 years in the making, would make new and more effective kinds of diabetes medication possible.

"Until now we have not been able to see how these molecules interact with cells," said Lawrence, from the Walter and Eliza Hall Institute of Medical Research.

"We can now exploit this knowledge to design new insulin medications with improved properties, which is very exciting."

Lawrence said the team's study, published in the latest edition of Nature, had revealed a "molecular handshake" between the insulin and its receptor on the surface of cells.

"Both insulin and its receptor undergo rearrangement as they interact -- a piece of insulin folds out and key pieces within the receptor move to engage the insulin hormone," he said of the "unusual" binding method.

Understanding how insulin attaches to cells was key to developing "novel" treatments of diabetes, a chronic condition in which the pancreas does not produce enough insulin or the body cannot use it properly.

"The generation of new types of insulin have been limited by our inability to see how insulin docks into its receptor in the body," said Lawrence.

"This discovery could conceivably lead to new types of insulin that could be given in ways other than injection, or an insulin that has improved properties or longer activity so that it doesn't need to be taken as often."

Importantly, he said it could also have ramifications for the treatment of diabetes in developing nations, allowing for the creation of a more stable insulin that does not need refrigeration.

There are an estimated 347 million diabetes sufferers worldwide and diagnoses are increasing, particularly in developing countries, due to growing levels of obesity and physical inactivity.

It is expected to be the seventh leading cause of death in the world by 2030, with the World Health Organisation projecting total deaths from diabetes will rise by more than 50 percent in the next 10 years.

Complications of diabetes include blindness, limb amputation and kidney failure.

Doctors in Britain looked at five studies in which children were monitored from infancy to early childhood to see if vitamin D supplements made a difference to the risk of becoming diabetic.

The risk of developing the disease was reduced 29 percent in children who took extra vitamin D as compared to those who had not.

Diabetes is a chronic condition in which the body does not produce enough of the hormone insulin, or cannot make proper use of the insulin it does produce, a condition called insulin resistance.

In Type 1 diabetes, so-called beta cells in the pancreas that produce insulin are destroyed in early childhood by the body's immune system.

The disease is most common among people of European descent, affecting around two million Europeans and North Americans, and for reasons that are unclear is becoming more widespread.

Type 2 diabetes, which is far more common, is linked mainly with an unhealthy diet and sedentary lifestyle. It is becoming epidemic in scale in many developed or fast-developing countries.

HELSINKI: Low glycogen levels could explain why some people always turn aggressive or violent when they drink, a Finnish study showed Monday, suggesting the problem could be avoided with medication and regular meals.

The University of Helsinki and Helsinki University Hospital study analysed the insulin and glycogen levels of 49 "impulsive, violent, antisocial male offenders" who had alcohol problems and committed violent acts when drunk.

Researchers compared their insulin and glycogen levels with a control group of 40 healthy men.

During the eight-year follow-up period, 17 out of 49 men committed at least one new act of violence while under the influence of alcohol.

Tests showed that insulin levels among the repeat offenders was higher and glycogen levels were lower than among men in the control group or those who did not commit violent offences again.

The human body converts carbohydrates such as bread into glucose and some of it is stored as glycogen in the liver and muscles.

When the blood insulin level is low, for example after a fast, the liver converts glycogen back into glucose and releases it into the bloodstream.

"This might suggest that substances increasing glycogen formation and decreasing the risk of hypoglycemia might be potential treatments for impulsive violent behaviour," researchers said in a statement.

They added that regular eating habits while drinking alcohol were also important in preventing new violent crimes. - AFP/ar

The trend, based on diagnosed cases between 1989-2003, will be highest in the former Communist countries of eastern Europe, it warns.

The paper, published online by the British journal The Lancet, says the increase is so dramatic that it cannot be attributed to genes alone.

Instead, "modern lifestyle habits" are the likely culprits, it says.

Diabetes, a potentially lethal condition, affects 246 million people worldwide and is expected to affect some 380 million by 2025, according to the International Diabetes Federation (IDF) website.

Type 1 diabetes generally occurs in childhood and early adolescence.
The immune system destroys cells in the pancreas that make insulin, the hormone that breaks down glucose into other forms of energy, and this causes sugar levels in the blood to rise dangerously.

Experts say the disorder seems to be caused by a mix of genetic vulnerability and environmental factors.

These include increases in weight and height, less exposure to early infections in childhood and delivery by caesarean section.

Type 2 diabetes, which affects far more people than Type 1, occurs when there is insufficient insulin or cells become insensitive to the insulin that is produced.

It is closely associated with chronic obesity, which has become an epidemic in the industrialised world as a result of sedentary lifestyles and the switch to sugary and fatty foods. — AFP
