Down with colds and flu

I wasn't able to do some posting of articles this whole week... I was down with colds and flu, and my coughing was very bad. I even had several bouts of high fever. I mean, it was Singapore's National Day, so the Friday before that, which is the 8th, I paid the company doctor a visit and asked for some medicine that I can take during the long weekend. Then I went out with my family and many others to the beach and even participated in some light games/competitions.

And that was in the morning hours.

When the sun was coming up higher and hotter, I felt that I wasn't getting any better.

True enough, that was when my fever first occurred. I had my medicines brought along, so I took some, then had to lie down to rest. I managed to get some winks.

But I didn't get any better after all.

Monday was holiday in lieu, so I had the whole day to rest.

Tuesday I went to work.

Wednesday, I still wasn't any better, even with all the pills and syrups I was taking. So at quitting time, I decided to go back to the company-appointed clinic, and the doctor gave me some stronger medicines.

I got better, and here's to say that I will be back, most probably by next week.

Hope that everybody else getting the bout of colds or flu would also be better. Drink a lot of water, and don't forget your medicines.

Rare flu-like virus on the rise

WASHINGTON: A rare virus has killed three people and sickened nearly 100 in Japan, the Philippines, the United States and the Netherlands over the past two years, US health authorities said Friday.

The culprit is human enterovirus 68 (HEV68), and its respiratory symptoms can be particularly dangerous to children, the US Centers for Disease Control and Prevention said in its Morbidity and Mortality Weekly Report.

In six separate clusters of the virus that showed up worldwide, patients commonly experienced cough, difficulty breathing and wheezing.

The highest number of cases were found in Japan, where local public health authorities reported more than 120 cases last year.

However, the CDC said it could only confirm clinical data for 11 of those patients, all children, one of whom died.

The Philippines had 21 cases in late 2008 and early 2009, causing two deaths, the CDC said.

Other cases surfaced in the Netherlands and the US states of Georgia, Pennsylvania and Arizona, for 95 total confirmed cases over two years.

The virus was first discovered in four children who were sick with pneumonia in California in 1962, but subsequent incidences have been rare and sporadic, according to the CDC.

"Identification of a large number of patients with HEV68 respiratory disease detected during a single season, such as described in this report, is a recent phenomenon," it added.

"Whether this increase in recognized cases is attributable to improved diagnostics or whether the clusters themselves represent an emergence of the pathogen is unknown."

The CDC said its report aimed to highlight HEV68 as "an increasingly recognized cause of respiratory illness" and urged clinicians to report cases of unexplained respiratory illness to public health authorities.

Human enterovirus is closely related to human rhinovirus, which causes the common cold.

Novel Vaccine Is Poised to Move Quickly Into Human Trials

In NIH-Funded Study, Researchers Say the Approach Could Also Stop Addiction to Other Drugs, Including Heroin and Nicotine

NEW YORK (Jan. 4, 2011) — Researchers have produced a lasting anti-cocaine immunity in mice by giving them a safe vaccine that combines bits of the common cold virus with a particle that mimics cocaine.

In their study, published Jan. 4 in the online edition of Molecular Therapy and funded by the National Institute on Drug Abuse, the researchers say this novel strategy might be the first to offer cocaine addicts a fairly simple way to break and reverse their habit, and it might also be useful in treating other addictions, such as to nicotine, heroin and other opiates.

“Our very dramatic data shows that we can protect mice against the effects of cocaine, and we think this approach could be very promising in fighting addiction in humans,” says the study’s lead investigator, Dr. Ronald G. Crystal, chairman and professor of genetic medicine at Weill Cornell Medical College.

He says the antibody immune response produced in lab mice by the vaccine binds to, and sequesters, cocaine molecules before the drug reached the brains of these animals — and prevents any cocaine-related hyperactivity. The vaccine effect lasted for at least 13 weeks, the longest time point evaluated.

“While other attempts at producing immunity against cocaine have been tried, this is the first that will likely not require multiple, expensive infusions, and that can move quickly into human trials,” Dr. Crystal says. “There is currently no FDA-approved vaccine for any drug addiction.”

“An approach that works is desperately needed for cocaine addiction, which is an intransigent problem worldwide,” he adds. “There are no therapies now.”

The novelty of this possible treatment is that it hooks a chemical that is very similar in structure to cocaine, onto components of the adenovirus, a common cold virus. In this way, the human immune system is alerted to an infectious agent (the virus) but also learns to “see” the cocaine as an intruder as well, Dr. Crystal says. Once the structure of the new intruder is recognized, natural immunity builds to cocaine particles, so any time cocaine is snorted or used in any way, antibodies to the substance are quickly produced and the cocaine molecules are engulfed by the antibodies and prevented from reaching the brain.

“The human immune system doesn’t naturally tag cocaine as something to be destroyed — just like all small-molecule drugs are not eliminated by antibodies,” he says. “We have engineered this response so that it is against the cocaine mimic.”

In this study, a team of researchers — scientists from Weill Cornell Medical College, Cornell University in Ithaca, and the Scripps Research Institute in La Jolla, Calif. — ripped apart an adenovirus, retrieving only the components that elicit an immune response and discarding those that produce sickness. They then hooked the cocaine analog on to these proteins to make the vaccine. “We used the cocaine analog because it is a little more stable than cocaine, and it also elicits better immunity,” Dr. Crystal says.

The researchers then injected billions of these viral concoctions into “garden variety” laboratory mice (mice that are not genetically engineered). They found a strong immune response was generated against the vaccine, and that these antibodies, when put in test tubes, gobbled up cocaine.

They then tested the vaccine’s effect on behavior, and found that mice that received the vaccine before cocaine were much less hyperactive while on the drug than mice that were not vaccinated. The effect was even seen in mice that received large, repetitive doses of cocaine. Proportionally, the cocaine doses reflected amounts that humans might use.

The vaccine needs to be tested in humans, of course, says Dr. Crystal, but he predicts that if it works, it will function best in people who are already addicted to cocaine and who are trying to stop using the drug. “The vaccine may help them kick the habit because if they use cocaine, an immune response will destroy the drug before it reaches the brain’s pleasure center.”

In addition to Dr. Crystal, the study’s authors included Martin J. Hicks, Bishnu P. De, Jonathan B. Rosenberg, Jesse T. Davidson, Neil R. Hackett, Stephen M. Kaminsky and Miklos Toth of Weill Cornell Medical College; Jason G. Mezey of Weill Cornell Medical College and Cornell University in Ithaca, N.Y.; Amira Y. Moreno, Kim D. Janda, Sunmee Wee and George F. Koob of the Scripps Research Institute in La Jolla, Calif.

They tracked 1,002 adults in Wisconsin aged 18-85 for 12 weeks in the autumn and winter of 2008, monitoring them for respiratory illness and weight and quizzing them about diet, lifestyle and aerobic exercise.

People who described themself as fit or who exercised up to five days a week or more, had between 4.4 and 4.9 "cold" days on average.

For those who fell in the middle category of fitness, and exercised between one and four days a week, this was 4.9-5.5 days.

But among counterparts who said they were of low fitness and who exercised only one day a week or less, the tally was between 8.2 and 8.6 days.

Good fitness also caused the severity of cold symptoms to fall by between 31 and 41 per cent lower compared with the most sedentary lifestyle.

Bouts of exercise unleash a temporary rise in immune defences, helping to boost preparedness against viral intruders, the study suggested.

It cited figures that the average adult in the US can expect to have a cold two to four times a year, and children between half a dozen and 10 colds a year. The cost to the US economy is put at around 40 billion dollars annually.

The paper, published in the British Journal of Sports Medicine, is headed by David Nieman, director of the Human Performance Laboratory at the Appalachian State University in North Carolina.

Addressing concerns on infections in children

All parents want the best for their children and constantly worry about their well being. Children who fall sick often miss out on learning opportunities and playtime which affects their overall growth and development. In this week’s column, A/Prof Daniel Goh helps parents to clarify uncertainties regarding infections in children.

Q1: Are there any vaccinations that my child should take? Are there any side effects to vaccinations?Evelyn Lee

A: Apart from the routine vaccines in our national immunization programme, there are several other vaccines that you should consider for your child. These include the rotavirus vaccine (given orally, only for babies less than 6 months of age), chicken pox vaccine (above 1 year old), influenza vaccine (which should be taken once a year), hepatitis A vaccine and the pneumococcal conjugate vaccine. Do speak with your family doctor who will be able to guide you in your child’s immunization needs.

Side effects of vaccines are usually mild and self-limiting and include pain, redness and fever. Allergy to vaccines is very rare. If your child has egg allergy, she should not receive influenza or yellow fever vaccines as these contain traces of egg. The measles vaccination is safe even in a child with egg allergy. If you notice that your child develops rashes, eye swelling or breathing problems after receiving a vaccine, you should bring your child immediately to your doctor for further assessment and management.

Q2: What are auto-immune diseases and what are the symptoms?Belinda Ang

A: Auto-immune diseases occur when the body’s immune system reacts erratically against itself and forms antibodies to attack healthy cells, tissues and organs. Some examples of auto-immune diseases include Lupus, Dermatomyositis and Type 1 Diabetes Mellitus. The symptoms of auto-immune diseases vary according to the type of disease. Some of the symptoms can be subtle and include persistent fever, weight loss, general tiredness and malaise, persistent or recurrent skin rashes, joint pain and swelling. Do consult your doctor should your child have such symptoms.

Q3: The doctor usually prescribes a course of antibiotics for a child to clear an infection. How does it actually work against the virus?Gloria Phua

A: Infections can be caused by bacteriae, viruses and other pathogens. Antibiotics are used to treat specific bacterial infections such as some ear and sinus infections, and some types of pneumonia. However, antibiotics have no effect on viruses at all and should not be used in viral infections such as the common cold and the  u. When prescribed, it is also important for you to take your antibiotics properly and to complete the course as instructed by your doctor. Overuse/misuse of antibiotics can lead to other complications. It can also cause the development of resistant strains of bacteriae that are even more difficult to treat.

A/Prof Daniel Goh is the head of both Department of Paediatrics and Division of Paediatric Pulmonary and Sleep at the University Children’s Medical Institute, NUH. A/Prof Goh also pioneered the “I CAN!” programme (a comprehensive programme for the management of asthma and allergies in children at NUH). He is the President of the Singapore Paediatric Society and the Vice-President of the College of Paediatrics and Child Health, Singapore as well as the Vice-President and President-elect of the Asean Paediatric Federation.

The flu is more common than you think
Influenza, commonly known as the 'flu', is a viral infection of the respiratory tract. Potentially severe, it is cause by several strains of the influenza virus and is easily transmitted through water droplets in the air. Once it infects a person, the virus replicates itself very quickly, making you feel sick in no time.

Flu seasons peak during the winter months in temperate countries. However, tropical countries such as Singapore do not have as well0-defined flu seasons. Although deadly strains of flu like the 'bird flu' or SARS follow a certain cycle of potential major outbreaks, flu in general is present all year round.

The flu is more contagious than you think
When an infected person sneezes, each sneeze travels at speeds of up to 100 km/h. A sneeze can produce about 20,000 tiny water droplets which can remain in the air for as long as 24 hours. Surrounding people unsuspectingly breathe that in and this is how the flu spreads so quickly.

The flu can cause severe symptoms
Although the severity may vary between individuals, generally the common symptoms are:

These symptoms are often more severe in the elderly or those with chronic illness, sometimes leading to secondary bacterial infections like pneumonia. The flu may also worsen heart, lung and other chronic diseases (e.g., asthma, chronic obstructive pulmonary disease, myocardial infarction and diabetes). Severe complications of flu infection may even lead to death in patients with impaired immunity. Deaths caused by the flu in Singapore over the past decade were 14.8 out of every 100,000 persons.

How to tell if you've got the flu
If you've got a fever (usually 39°C – 41°C) and 2-3 other symptoms mentioned above (most commonly muscle aches and cough), you've probably got the flu.

You can prevent the flu
Vaccination is one of the options to prevent flu. Vaccination of the elderly can result in lower incidence of complications and hospitalizations. Death rates due to severe complications in high risk individuals have been shown to be reduced by as much as 68%.

The importance of routine vaccination
The flu virus mutates and changes continuously. Throughout the year, 110 national flu surveillance centres and 4 World Health Organization (WHO) collaboration centres in 82 countries worldwide watch the trends in circulating strains of influenza. The WHO will regularly identify the strains that most resemble those in circulation. Shortly after the WHO releases this report, vaccine manufacturers will start making new vaccines to offer protection against these strains of influenza. This is why routine vaccination is important for optimal and continuous protection against new flu strains.

Who should be vaccinated
Anyone who wishes to reduce the risk of getting the flu can be vaccinated. It becomes even more important to do so if the following applies to you or a family member:
  • All persons, including school-aged children, who want to reduce the risk of becoming ill with influenza
  • All persons aged 50 years and above
  • All children aged 6 months to 4 years
  • Persons with chronic diseases such as asthma, cardiovascular diseases and diabetes
  • Close contact with high risk groups
  • Have low immunity
  • Traveling to high risk areas
  • Resident of a nursing home or chronic care facility
  • A healthcare worker

We caught up with noted paediatrician, Dr Daniel God (NUH), to help moms gain further insight into strengthening their child’s immune system.


Q: My 3 year old loves playing outdoors. However, he’s prone to colds and flu so I often restrict his playing to prevent him from falling sick. Could his immune system be weak?Elaine Phua
 
A: Colds and flu are due to viruses that commonly cause infections in young children, particularly those attending childcare or playschool. Playing outdoors is not the cause of him falling sick. In fact, keeping your child active and ensuring he gets lots of exercise and sunshine helps to keep him healthy and strong. After play, make sure he drinks lots of water to replenish lost fluids and gets sufficient rest.


Q: I’ve heard from other moms that prebiotics play an important role in supporting my child’s gut health. What exactly do prebiotics do for my child?Karen Ang
 
A: To maintain a healthy environment in your child’s gut, your child needs good bacteria in his intestines. Good bacteria need nourishment in the form of prebiotics, which help to stimulate the growth of good bacteria in your child’s gut. Prebiotics also play a protective role in the gut by promoting a thicker mucous lining, which acts as a barrier against the build-up of bad bacteria.

Ensure your child gets his share of prebiotics by feeding him foods such as bananas, honey and whole grains.


Q: How do I know whether my child’s immune system is strong enough? What can I do daily to strengthen his immune system so he can be stronger and healthier?Debra Ong
 
A: You can boost your child’s immunity in several ways: Ensure that he gets a well-balanced, nutritious diet, so that he gets all the nutrients he needs. Also, keep your child active and lean as it has been clinically proven that overweight children are more likely to suffer from infections compared to lean babies.

Getting adequate rest and sleep are also vital to allow the body to recuperate and meet the needs of the day. Good personal hygiene is also important to reduce unnecessary exposure to infection. Finally, be sure to check with your doctor if your child has received all the necessary vaccinations against diseases and infections.
