For joint replacements, like a knee or hip, is muscle attached to the artificial implant?

Ordinarily “no muscle attaches to any hip or knee implant,” said Dr. Mathias P. Bostrom of the Hospital for Special Surgery in Manhattan. The only human tissue that adheres to an implanted joint is bone.

The link is made either with cement or directly to the bone. When the connection is direct, the bone-producing cells grow new material that forms a bond with the implant, a process called osseointegration.

“A lot of knee implants use a kind of cement, really more of a grout, that fixes to the bone itself,”
 Dr.Bostrom said. “It is an acrylic, similar to Plexiglas, called PMMA, for polymethyl methacrylate.”

In the case of a tumor prosthesis for a bone like the femur – in which a lot of bone is removed and large segments are replaced with metal – there may be some linkage of muscle and tendon to implant, he said.

In most joint implants, however, the muscle is still attached to the bone via the tendon, and the surgeon especially tries to avoid detaching any muscle insertion in the hip or knee.

“We want the tendon’s attachment to the bone to be intact,” Dr. Bostrom said.

Want better sex? Consider getting a new hip or knee.

Before Mary Ann Oklesson, a New York magazine publisher, had both hips replaced a few years ago, the pain of arthritis made it difficult to walk, exercise or even climb into a taxi. Her failing hips had also taken a toll on her sex life.

"Sex was somewhat painful," said Ms. Oklesson, now in her early 60s. "If I had to pick my leg up to put it in a cab, you can imagine what sex was like."

But all that changed after hip replacement surgery. "It definitely improved my quality of life, and my love life," she says.

While researchers have long known that hip and knee replacement leads to less pain and improved mobility, new research shows that the surgery offers an unexpected bonus in the bedroom. Among 147 patients who had joint replacement surgery in New York, most said arthritis had interfered with their sex lives, according to research presented in March at the annual meeting of the American Academy of Orthopedic Surgeons. After surgery, 81 percent of those patients who had said their sex lives had suffered as a result of a bad joint reported that the frequency of sexual activity had increased.

Many reported an increase in libido and stamina and an improvement in their ability to climax. The benefits were specially pronounced among patients whose complaint had been failing hips, as well as women, who reported the most discomfort during sex because of painful joints.

"If achieving a position in sexual function is very uncomfortable, it's unlikely to be fruitful in terms of achieving climax," said Dr. Jose A. Rodriguez, director of the Center for Joint Preservation and Reconstruction at Lenox Hill Hospital in New York and lead author of the study.

Hip replacement surgery has risen 85 percent in the past decade, with doctors in the United States performing more than 300,000 procedures in 2010, the latest year for which statistics are available. Much of the increase has been fueled by active middle-age adults, 45 to 65. In that age group, hip replacements have nearly tripled to 128,000 during the same period.

Dr. Claudette Lajam, an orthopedic surgeon at the Center for Musculoskeletal Care at New York University Langone Medical Center, said that so many patients have questions about intimacy after joint replacement that she had added a page to her website devoted to sex.

"That page gets the most hits of any page on my website," Dr. Lajam said. "There are a lot of people who get back out there, or get closer to their spouse, because they've been unable to participate in that intimacy for a while. Just the relief of pain itself improves the relationship."

That was the case for D'Arcy Achziger, an apparel sales director in New York who opted for knee replacement two years ago after suffering from excruciating pain.

After surgery, "my husband was thrilled," said Ms. Achziger. "I had been cranky, just so crabby because it hurt. I was much more pleasant after surgery, and it made his life so much nicer."

Dr. Charles Cornell of the Hospital for Special Surgery in New York, said some patients are hesitant to talk about the toll joint pain can take on their sex life.

"It's especially important to our younger patients," he said, "but believe it or not, I've had patients in their 80s who this has been a topic for."

Females more prone to knee injury in football

WASHINGTON: "Kicking like a girl" is a real phenomenon and may explain why females are more likely to suffer knee injuries in sports such as football, US researchers said on Wednesday.

The researchers found significant differences in knee alignment and muscle activation between men and women while kicking a ball.

The study appearing in the Journal of Bone and Joint Surgery offers a possible explanation on why female players are more than twice as likely as males to sustain an anterior cruciate ligament (ACL) injury.

"By analyzing the detailed motion of a soccer kick in progress, our goal was to home in on some of the differences between the sexes and how they may relate to injury risk," said orthopaedic surgeon Robert Brophy of the Washington University School of Medicine in St. Louis, Missouri, and the study author.

"This study offers more information to help us better understand the differences between male and female athletes, particularly soccer players."

Brophy and his colleagues from the Motion Analysis Laboratory and Sports Medicine Service at the Hospital for Special Surgery in New York used 3-D video-based motion analysis and electromyography to examine the differences between 13 male and 12 female college soccer players during the action of kicking a soccer ball.

They found that male players activate the hip flexors inside the hip in their kicking leg and the hip abductors outside of the hip in their supporting leg more than females. The males also generated more than twice as much activation of the gluteus medius and vastus medialis muscles on the upper leg and pelvis area.

"Activation of the hip abductors may help protect players against ACL injury," said Brophy, a former collegiate and professional soccer player.

"Since females have less activation of the hip abductors, their hips tend to collapse into adduction during the kick, which can increase the load on the knee joint in the supporting leg, and potentially put it at greater risk for injury."

Brophy said the study does not establish a cause-and-effect between muscle activation, knee alignment and ACL injuries, but added the data "moves us toward better understanding of what may contribute to differences in injury risk between the sexes and what steps we might take to offset this increased risk in females."

Football or soccer is one of the fastest-growing sports in the United States with approximately 20 million registered players, with a large number of them females.
